It's a combination of things that lead to Toriel's slow reaction: Slight disorientation from the sudden position swap (nothing major), shocked staring at the blasters (she had no idea), and bracing herself when it looks like Croc is charging again.
And then he's jumping up.
It hits her shade of a second too late what he's doing and she's just far enough away from Sans that she can't knock him out of the way. Not that she doesn't try. Heart stopping, she manages only a step...
